Condition Help

Special Support to Prevent Emergencies

Condition Help is a patient- and family-centered care initiative that allows patients or their families to activate a team to address immediate clinical and safety concerns.

When to Call Condition Help

Call for help if:

  • you notice a change in the patient’s condition and the health care team is not addressing the concern
  • you feel there is a breakdown in how care is given
  • you think the treatment or medicine is different from what was ordered
  • you have concerns about a medicine

Condition Help is not appropriate for concerns regarding:

  • patient meals
  • TV and telephone service
  • room temperature
  • housekeeping
  • other non-clinical matters

Don’t be afraid to call. Staff members will not get in trouble. Our goal is to help patients and their family members become part of the care team. Contact your nurse if you have questions.
